The High 5s project is a patient safety collaboration among a group of countries and the WHO Collaborating Centre for Patient Safety in support of the WHO Patient Safety Programme. Within this initiative a standardized Medication Reconciliation process (High 5s SOP Medication Reconciliation) is being tested in hospitals in Australia, France, Germany, the Netherlands and the USA. After completion of the project in 2015 the finalized SOP Medication Reconciliation will be available for widespread dissemination. More information on the High 5s project can be found here:
