Avoid radiology scans on pregnant patients based on the WHO´s recommendations for Patient Safety and those from the Spanish Ministry of Health.

Methods:

- Root cause analysis of an incident at Malvarrosa´s Hospital.
- Detection of reasons that influenced the occurrence of the incident.
- Description of barriers to avoid performing radiological explorations to pregnant women.
- Identify barriers to implementation — and strategies to overcome them:

For 1st Barrier: Develop a "Professional protocol to avoid incidents and adverse events in interventions carried out in the Radiodiagnpstic Service and in Services where radiographs are performed". The physician requesting radiographic examination should:
- Give information to the patients about the exploration they will be involved in.
- Explain the reason that prompts such exploration
- Provide informed consent if applicable
- Ask if the patient could be pregnant and write it down in the medical history.
For 2nd Barrier: Posters
Two types of posters will be placed: one from the Spanish Radiophysics Society and another one designed in the Malvarrosa´s Hospital (Annex I)
For 3rd Barrier: Check-list for the staff involved in the procedure (Annex II)

IMPLEMENTATION:

- Obtain the support of the heads of the Radiodiagnostic departments and the heads of other departments where radiographs are performed
- Distribution of posters and cheklist by the Quality Unit

EVALUATION:
- Twice a year
