Aim and the benefit of the Patient Safety Practice
 
The aim of this "At Home Care for Mental Health" program is to provide intensive care of the crisis in the daily life environment of the patient using as little restriction as possible and enhancing patient and families’ responsibility and control. Expected benefits are the same clinical results, enhanced patient and families’ satisfaction and smaller costs than providing the crisis care in the hospital ward (48 vs 387 Euros per inpatient day in 2013).
Description of the Patient Safety Practice
 
Patients requiring intensive care because of a critical situation impossible to manage in an outpatient setting are selected in the emergency ward, included in the program and visited at home by a psychiatrist and a specialized nurse instead of being placed in hospital ward. Patients having gone through a first period of inpatient care and who are not with the required autonomy for outpatient care are also eligible for the program aiming to shorten the hospital stay. A contract is signed by patient and family. The patient clinical status and patient and family's satisfaction are monitored. The main difficulties encountered during the implementation of the practice have been: to arrange professionals' displacements and to provide 24 hours support to patients and families. In order to solve these difficulties: 1) An agreement with a taxi company has been established by the health authority to facilitate the displacements; 2) A mobile telephone number managed in labour time by the program staff and the rest of the time by the nursery of the inpatient ward – who has access to the crisis plan - is available for patients and families 24 hours.
