Aim and the benefit of the Patient Safety Practice
 
In order to provide evidence based care that is safe and respectful of patients' rights, we decided to summarize in the Manual for development of the Individual Care Plan, all the recommendations and good practices related to care and rehabilitation so that the each professional, working in the nursing homes of Emilia Romagna Region, has a guidance in terms of appropriateness and effectiveness of the interventions envisaged. The Manual addresses the issue of safe management of drugs together with the Ministerial Recommendations regarding prevention of falls, prevention and treatment of decubitus ulcers, recommended practices regarding management of urinary catheters in order to reduce infections and sepsis, practices regarding infection control.
Description of the Patient Safety Practice
 
The study group started developing the Manual in 2011 and completed it in sprign 2012, through the following steps: 1. Audit on documentation related to care and reabilitation and identification of possible improvement areas or deviations from the good practice standard; 2 comparison and discussion on the meaning of the Individual Care Plan (ICP) as a tool for team integration; 3. Analysis of profiles and responsibilities of those involved in drafting the ICP as well as analysis of specific National and Regional legislation; 4. Definition of how to evaluate results; 5. Reading Ministerial recommendations on patient safety; 6. Research of nursing and rehabilitation-sensitive indicators; 7. Drafting the manual. Results show that the Manual is being used by 95% of the professionals
