Aim and the benefit of the Patient Safety Practice
 
At the Santa Maria del Carmine Hospital in Rovereto are hospitalized every year more than 200 patients for hip fracture, with an average age of 83 years. The normal activity of geriatric consultation at the OU of Orthopaedics cannot always give adequate coverage to the specific needs of these patients, often with co-morbidity and exposed therefore to higher risk of complications. Our orthogeriatrics program is aimed at improving the appropriateness and safety of care introducing tools of Multidimensional Geriatric Evaluation. The implementation of these tools,in fact, will improve the recognition of comorbidity and it will promote an integrated management approach for the treatment of orthopaedic patients with hip fracture (and other orthopaedic conditions). This can help to reduce the risk of complications ensuring safety, effectiveness and timeliness of care
Description of the Patient Safety Practice
 
It was established a orthogeriatrics program through educational events, and coordinated implementation of new tools and related practices. We adopted tools for multidimensional geriatric evaluation and integrated management of geriatric patients with hip fracture (mainly), allowing effective and timely recognition of risk factors, comorbidities, clinical status and early warnings of deterioration of patients. We identified predictors of disability and mortality in patients and we monitored quality of care and surgical complications before and after the beginning of program. Indicators showed improvement (see the attached document)
