Aim and the benefit of the Patient Safety Practice
 
Farmamemo is a project aimed at improving the safety of patients in Oral Anticoagulant Therapy (OAT). The Practice aims at preventing errors during drug administration with special regards to patient under oral anticoagulant treatment. The OAT requires frequent monitoring of INR with specific blood test to check the progress of health status of the patient and to update the treatment plan. Furthermore, to ensure the success of the therapy, the patient must follow some precautions relevant to her/ his lifestyle.
Farmamemo involves the use of specific organizational, technical and communication instruments aiming at the prevention of risks associated with OAT and to ensure that patients have high standards of quality in the management of therapy.
These tools require the active involvement of patients and physicians
Description of the Patient Safety Practice
 
In order to implement the practice the organization:
- has adopted a formal organizational model for the management of OAT
- has included the topic in training plan and has identified the hospital staff, local and general medicine staff, concerned with the management of OAT, to be involved in the mandatory ECM training
- has planned patient education by providing the necessary support materials; has
established a formal agreement with the doctors and the structures involved (districts, laboratories, units of primary care etc..) for the monitoring of the TAO, the update of the therapeutic program, the periodic access or need for revisions clinical
- has a program and tools for patient education consistent with the contents of the handbook for the patient in OAT developed by the Centre GRC.
- Patients in OAT have a dedicated path for monitoring INR and for updating the treatment plan that includes:
execution of the withdrawal within 12h
sending the reply to the doctor in writing
within 24 hours from the execution of withdrawal
updating and delivery to the patient's treatment plan in writing,
programming in writing of the date of the next INR control
Health care providers ensure access to TAO patients for clinical review periodically and as needed.
-The organization uses hardware and software tools and proven reliable usability for the determination of the INR and the transmission of results
-Patients receive the right information in a clear and unambiguous on the management of TAO in the territory of residence
-Patients have easy access to the levy and the surveys, they receive the update of the therapeutic program in writing before the next dose (within 24 hours of collection).
