Aim and the benefit of the Patient Safety Practice
 
Our clinic is specialized in treating diabetic patients. We encountered sometimes cases of diabetic patients with psychiatric problems (depression or dementia associated / psychosis) that had not been diagnosed until been admitted in our clinic. These kinds of patients can develop an aggressive behavior towards self or to the medical staff or other patients, or simply their adherence to prescribed treatment was very low. We felt that these problems can be solved by creating a filter type of psychological testing to help us detect potential mental patients. Benefits to diabetic patients with severe depression or dementia / psychosis are: prevention of dangerous actions, aggressive behavior or suicide attempts. We obtained also an increase in patient compliance to treatment.
Description of the Patient Safety Practice
 
The practice is design to timely detect the patients with double diagnosis: diabetics and psychiatric disorder in order to assure the psychological evaluation and assignment to psychiatric treatment.

The nurses and health care assistants are spending a long period of time with the patients and they can easy observe possible behavioral abnormalities.

The chief of department and the team established a practice to observe and select the patients and to inform the coordinating doctor about possible abnormalities.

The main activities of the practice are:
-    The nurses and health care assistants have been instructed for this purpose and they applied with positive results.
-     Patients with psychological problems are identified by medical staff during the process of daily medical care within hospital.
-    Nurses and health care assistants inform the coordinator doctor which are the patients with possible psychological problems and he is referring to hospital psychologist within 24 hours
-    The psychologist evaluates the cases and recommends the psychiatric consultation for those in need.
-    The coordinator doctor together with psychiatrist is following up the patient evolution.
-    The coordinator doctor and the nurse educate the patient to understand the importance of respecting the treatment, the diet and the life style; the patient is receiving also written information materials to use at home for taking the medicines, preparing the meals, avoidance the risk factors, etc.

Results:
-decreasing the number of events: self aggression, medical staff aggression, suicide attempts
-improving treatment adherence of the patients
