Aim and the benefit of the Patient Safety Practice
 
Patient safety is a priority for the "hospital of Huelva". One of its strategic lines of safety-related activities includes patients. The errors in medicine are a major cause of damage to the patients. The goal of this guide is to help hospital leaders better understand how to address the issue of patient safety. Included are a set of measures that provide a perspective on hospital quality of care to reduce errors and increase patient safety. The list is divided into three parts that correspond to the steps of the pathway of care (before admission, admission and discharge)
The guide can help hospital leaders:
• To foster a supportive culture for safety of professionals
• To improve reporting of medical errors
• To monitor patient safety routinely
Description of the Patient Safety Practice
 
The practices selected include six objectives related to Patient Safety on the basis established by the World Health Organization and the Hospital Accreditation Program.
It is a reminder tool that helps professionals to meet the challenge of clinical safety, in order to minimize risks of inpatient care practice. In addition, this list includes the main actions on hospital patient safety, which could be considered in any hospital setting:
GOAL 1:IMPROVE THE ACCURACY OF PATIENT IDENTIFICATION.
 - Use at least two patient identifiers when providing care, treatment, and services.
- Identifying patients. Identify the high-risk patient: mental disorder, disabled
GOAL 2: COMMUNICATION:
 - Communication between professionals: Verbal orders, handovers
GOAL 3. CORRECT MEDICATION
 - Avoid medication abbreviations
 - Check: connecting dose and administration route.
 - Register expiry dates. Identify high-risk medications
GOAL 4. WASH HANDS
 - Use hydro-alcoholic solution before and after procedures
GOAL 5. SAFE PRACTICES
 - Performing the correct procedure at correct body site.
 - Avoid connection errors
 - Using catheters and tubes once
 - Confirm injection devices
GOAL 6. REPORT
 - Report: adverse events, drug allergy reactions.
 - Report pressure ulcer.
 - Register falls
- Analyze and evaluate improvement measures
