Identifying and reducing the risk of drug interaction during polypharmacy. The main target lays in the identification of the drug interactions risk in the population, in order to supply the correct information to General Practitioners with a preventive and corrective goal.
Description of the Patient Safety Practice
 
The project aims to identify the risk of drug interactions in polytherapy patients and to prevent this risk through a warning to the doctor. We analyzed the drugs prescriptions data (data base: DWH, BDA and File F) of all local people who were taking more than two drugs chronically. In several cases we recorded more than one type of interaction in the same patient.The data obtained were crossed with the database on pharmacological interactions, as well as to identify patients at risk of interaction. Whereupon it has been processed, for all physicians, a medical report. Every general practitioner received a report, for each patient at risk of interaction, in which for every copy of drugs there is:
- the type of interaction expected;
-the severity of this interaction (contraindicated, major, moderate and minor);
-how the interaction is documented in the literature (excellent, good, fair, poor documentation);
-the time in which it can be expected that interaction (immediate or delayed);
-how to prevent or treat this interaction.
The identification of patients at risk of interaction and reporting to physicians leads to the re-evaluation of current therapy and to its possible modification or suspension.
