Description of the Patient Safety Practice

Prevalence studies and data on the use of advanced dressings (in 2003) highlighted the fact that there is limited attention given to the detection of the risk of decubitus lesions, and an inadequacy in their treatment. For this reason, a multi-professional and multi-disciplinary working group has drawn up guidelines for the region for the prevention and treatment of decubitus lesions, and for anti-decubitus surfaces. These documents have been implemented at company level - initially at hospital level and subsequently to the rest of the territory - through training to all health care workers (with lectures, field training, and briefing during regular meetings of coordinators or managers from institutions, departments, and operational units) and information initiatives (using the in-house magazine, the magazine of the Provincial College of Nurses, the local press, and the company website). The monitoring carried out after the implementation phase has shown greater attention by the carers to the problem: In 95% of cases, patient files include an evaluation of the risk of lesion (Norton Stotts Scale) in all Operational Units and RSA (nursing homes); Prevalence studies indicate that many lesions are at an early stage (unlike the first survey that showed only chronic lesions, or those at an advanced stage); Improvements in the proper use of advanced dressings, and reduction in the use of obsolete products and systems; Efficient use of anti-decubitus aids.
