Aim and the benefit of the Patient Safety Practice
 
The problem of patient falls during hospitalization is of paramount importance, especially for the elderly people. As a matter of fact, after a fall the elderly, in addition to physical injuries, develops a sense of insecurity that force him/her to limit physical activity, thus jeopardizing their autonomy and quality of life. The project aims to improve safety of hospitalized patients through a system for assessing the risk of falls when admitted and re-assessing it during hospitalization. Furthermore it is aimed at implementing interventions for preventing the risk of falls.
Description of the Patient Safety Practice
 
The fall prevention program is made up of various steps.
A. booklet / poster face for users
B. identification of patients at risk for falls according to specific rating scale
C. environment evaluation through special checklist
D. fall reporting

Protocols for prevention of falls risk were examined by the working group basing on the scale of Conley , tehta showed a sensitivity of 71% ( the percentage of true positives , that is, those deemed at risk and actually fallen ) and a specificity of 59% ( the proportion of true negatives , that is, those not deemed to be at risk and not actually fallen ) . It was considered necessary to use the scale of Conley without any changes in the whole hospital with the exception of the Unit in the Department of Rehabilitation and Palliative Care for which was introduce a modified scale of Conley. The inclusion criteria for application of these scales are: - alert patient - patient hospitalized for more than 24 hours. The rating scale should be used as follows : - within 24 hours - whenever clinical conditions yor mobility ou change - after every fall - at discharge / transfer of the patient The evaluation is carried out by the nursing staff . In addition to the patient education programs tend to increase the knowledge about the environment , to increase self-confidence in own abilities and thus reducing the fear of falling At this aim information materials have been prepared and distributed in Hospital Units and affixed in patient areas . A checklist has been prepared to assess environmental risks. The cards is periodically checked and controlled by the Risk Management Group in order to identify useful information . In 2011, the procedure has been revised and supplemented with particular attention to pediatric patients , Prevention and containment of risk of fall is also supported by educational intervention for families. The practice is still on going; the evaluation process has been defined on the basis of the following indicators: 1. Patients who experienced with respect of total number of days in hospital 2. Percentage of patients with with respect to the number of discharged patients 3. Percentage of patients with poor prognosis with fall / severe 21 days. and / or death 4. Percentage of cards with fall risk assessment reports with respect to (total) reported falls.
