Aim and the benefit of the Patient Safety Practice
 
Medication errors are usually classified according to the stage of the proceedings in which they occur. The errors, however, occur at all stages of drug management. Medication errors can be reduced if all professionals work together as a team. Teamwork, however, is often a different way of working. A monitoring system has been introduced for the management process of the drug, with the active involvement of both medical and nursing staff of the Institutions. The pharmacist, the nurse coordinator, and a physician perform site inspections for wards and services where drugs are used to verify adherence to standards specified by the checklist. The checks are carried out on the basis of a predefined checklist which consists of 112 items. The results are communicated to the doctors and nurses of any institutions at the end of the audit, together with the proposed corrective actions. In departments where the audit finds major problems, training is provided to all staff.

The checklist examines the path of the drug starting from its request, handling, maintenance and storage, to disposal of expired medicines and their return to the pharmacy, management and storage of high risk drugs, management and preservation of free samples of medicines, personnel-patient medication management, proper prescribing, preparation and administration of drugs (including the rule of 6 G: right patient, right medication, right dose, right route, right time, right registration )
