Aim and the benefit of the Patient Safety Practice
 
Netzwerk CIRS-Berlin is a regional patient safety incident reporting system. It consists of a central database that is publicly available on a website (www.cirs-berlin.de), and hospital-specific (internal) reporting groups from currently 19 hospitals in Berlin. Therefore, it aims at improving inpatient patient safety in general and patient safety culture, too.
Description of the Patient Safety Practice
 
The Network CIRS-Berlin is a regional reporting system that consists of a central database that is publicly available on a website (www.cirs-berlin.de) and hospital-specific (internal) reporting groups from 19 hospitals in Berlin. Near misses are reported to the internal reporting systems. From these, reports are then transferred to the central system whenever the local administrators of the internal reporting system consider the report to provide a good learning opportunity for other hospitals.
Incident reports are primarily analysed within the hospitals, hospital representatives also collectively analyse them at bimonthly meetings of the network. During these meetings, measures to prevent similar incidents are developed. Information about incidents and preventive measures are communicated via monthly newsletters in order to inform staff in the member hospitals.
The practice started in 2008 and has been qualitatively evaluated in 2012. Results will be published in 2013. As evaluative methods the database of reports has been analysed, and representatives of the member hospitals surveyed by a written questionnaire and in-depth interviews with a focus on strengths and weaknesses of the network.
