Clinical safety is a key element in the process of care. The increasing complexity of health systems requires an integrated risk management in the field of mental health as well.

In our organization we have designed a program involving clinical safety and integrated cross so all assistive devices, hospital and community. The objectives of the program are:
- To develop in our organization the concept of "clinical safety" as a priority
- Develop protocols, procedures and intervention recommendations based on scientific evidence, to standardize safety practices
- Develop tools for recording the risk assessment
- Promote the use of methods of analysis of adverse events
Description of the Patient Safety Practice
 
Method:
- Management leadership in the development, dissemination and implementation program
- Defining mental health risks
- Definition of systematic risk assessment in mental health
- Design intervention protocols associated with each risk
- Conducting training activities aimed at enhancing the safety culture
- design software for recording the risk assessment

Intervention:
- Integration of risk assessment in the comprehensive assessment of patients
- Implementation of intervention protocols associated with each of the risks identified
- Standardization of safe clinical practices in different organizational devices
