Description of the Patient Safety Practice
 
Working groups on quality, constituted by clinical professionals from primary care and quality experts, have been established for different subjects (pharmacology, emergency room...). Regular meetings are arranged in order to standardize and establish similar protocols and clinical pathways in all primary healthcare centres such as:

- Emergency care (AMI, stroke) and basic life support protocols in adults, children and neonates. They should be visible in the emergency room (posters) and they must be selectively disseminated among all the professionals in the primary centre: basic protocols or specific protocols.
- Establish basic and similar contents regarding equipment, medicines... for all emergency rooms in all primary healthcare centres.
- Regular inspection of drugs expiry dates (once a month) and daily check-ups of equipment and materials both for emergency rooms and for emergency visits. Common checklists for monthly, daily or after use revisions.

In order to improve the quality of prescription, specific criteria START & STOP to avoid side effects are used, particularly in those patients taking five or more medications for more than three months. Each case is studied by a team of 3 people, alternatives are considered and a proposal is put forward for the professional to discuss the convenience of possible changes in the treatment.

The implementation is gradually taking place and future audits wll be performed for assessment.
