A lot of our employees feel insecure when they use a computer and operate in the reporting system. The aim was to improve the willingness to report an adverse event to the national data base for Adverse Events, and to improve the quality of the reports, so all relevant information are included in the report. This PSP is realted to PSP number 341
Description of the Patient Safety Practice
 
We had two types of pocket cards made, one for the reporter of adverse events informing about how to report, and one for the persons who analyze and process the reports in the database for adverse events.
The cards were introduced in a workshop where all the persons with responsibility for analyzing AEs were invited.
At the end of the workshop we invited participents to evaluate the cards and after the workshop we surveyed the spread and use of the cards e.g. we observed an increased number of reports to the database and a higher quality of the reports, which made the risk management process easier and sharing of learning based upon higher quality information
