Patients elder than 65 years admitted into acute medical and neurological units are all screened for their risk of falling. We use the instrument STRATIFY Risk Assessment Tool, which covers patients recent history of fall, agitation, visually impaired, frequent toileting, transfer and mobility (please see; http://www.ahrq.gov/legacy/research/ltc/fallpxtoolkit/fallpxtool3g.htm) if the total score is above 3 point on a scale from 1-5, we score again every week. We have a lot of fall preventing actions which are launched individually (footwear, ringing the bell for help, physiotherapy to educate and train patient etc.). There are new results from research which shows that screening has no impact and that is more important to pay attention to why the patient falls and prevent new falls. So we’re currently working to change our focus from risk assessment to finding the causes of falling and then taking this as a starting point of improvement.
