Method: The Global Trigger Tool (GTT). The GTT was applied to randomly selected medical records of individuals who had been discharged from the hospital. Ten randomly selected clinical records were chosen every fortnight. An internal team of two registered nurses and a physician reviewed the records, applying the GTT toolkit. The same team analyzed electronic records throughout the study using a standard procedure that took 20 minutes or less for the reviewers according to the GTT. Using the three modules (care and medication) of the GTT and including as triggers informed consent (blood transfusion and surgical consents) and surgical antibiotic prophylaxis all adverse events (AEs) were assigned to broad categories: infections, falls, medication errors, nursing care (pressure injuries, diaper dermatitis, skin tears, phlebitis), surgery, diagnostic procedures, and discharges. The NCC MERP index was used to evaluate severity. A Likert scale was used to know the preventable events. Number needed to alert (NNA) was defined as the number of triggers needed to be reviewed to detect one AE. Implementation: Since 2007 Evaluation: Through annual memory.
