Aim and the benefit of the Patient Safety Practice
 
We want to improve the correct patient identification during all the hospitalization process, making sure it is correct and visible at all times, and avoiding all the possible risks due to lack of identification such as blurry bracelets, error in diagnostic tests, drug prescription and administration, physiotherapy, occupational therapy and speech therapy.
Description of the Patient Safety Practice
 
According to the hospital procedure, any changes related to inpatient identification must be written on the bracelet immediately after it occurs (such as unit or bed change). There is a weekly review of all inpatient bracelets, checking that the data are correct and clearly visible (name, surname, bed number, patient record number, date of birth). Bracelets must be immediately replaced in case the labeled data do not match exactly with the patient, due to unit change or any other circumstances. The admission service must be informed of the bracelets to be issued and replaced if needed. This must be done in 100% of patients. There is a weekly control of the number of inpatients whose bracelet is in perfect conditions, as well as the number os inpatients who require bracelet replacement or those patients who, for some reason, do not have identification bracelet.
