Patient safety is an issue almost unknown to most of the citizens. Therefore, the objective of this PSP was to introduce a specific check list to evaluate Patient Safety in Hospital from the citizens' perspective. This allows more people to be involved in Clinical Risk Management topics, resulting in a global improvement of patient safety.
Description of the Patient Safety Practice
 
Following a participated assessment of patient centeredness with the involvement of citizens, promoted by Agenas since 2012, it was necessary to extend this assessment to issues concerning patient safety. The original check list for assessing patient centeredness, composed of 144 items, was integrated with 14 specific items, through which the representatives of citizens, along with health care professionals, can assess the level of patient safety in a particular facility. The purpose is to identify areas for improvement, in order to allow continuous growth of the system. The check list for patient safety was tested and evaluated in September, at the Nuovo Osepedale di Prato - S. Stefano, and is now available for use in all hospitals. It is made up of items aimed explore the extent to which the organization is committed to: preventing healthcare associated infections , using the Surgical Safety Checklist, incident reporting, improving communication with patients involved in an adverse event and their families, providing information to patients on risks and related safety measures in place within the organization, correct patient identification, measures to manage the risk of fall.
