This experience will involve drafting a "safety plan" at insitiution level for the prevention of adverse events with serious consequences from both "errors" related to drug therapy (according to what is stated in the Recommendation. No. 7 of the Italian Ministry of Healh - March 2008) "errors" due to "LASA" medications (in accordance with what is stated in the Recommendation No. 12 of the Italian Ministry of Health - August 2010).
Description of the Patient Safety Practice
 
Based on the Recommendations issued by the Italian Ministry of Health and taking into consideration documents and procedures already in place within the institution, an ad hoc procedure ("safety plan") regarding safe management of medicines was drafted by the hospital management and the quality manager. This is currently being implemented: it has been distributed to all the people involved in medication management, and then its application will be verified through internal audits. Areas of application: all medications especially "high risk" or "high level of concern" because of their potential toxicity, low therapeutic index and high possibility of interactions. Most errors occur in the prescription phase and concerns Hypoglycemic (28.7%), cardiovascular (18.6), anticoagulants (18.6) and diuretics (10.1%) and involves inpatient care. The results we are going to measure are related to the decrease in inappropriate medication therapy, through measuring indicators of adverse events (both actual and potential).
