Aim and the benefit of the Patient Safety Practice
 
Based on Safety WalkRounds™, “VI.SI.T.A.RE” is aimed at listening to and supporting frontline staff in wards about safety issues in order to develop an open culture where patient safety is seen as the priority of the organization and improvements can be identified.
Description of the Patient Safety Practice
 
We developed an innovative tool putting together patient safety and staff safety and integrating in the same group executives of workers safety, patient safety and risk management. The main strengths of VI.SI.T.A.RE project lie in: • promotion of integration between the safety networks (of patients and workers) through the formation of a corporate group; • enhancement of ethical approach (transparency, open communication between professionals, enhancing the contribution of each professional); • checking application of Ministerial recommendations for patient safety and best practices for worker safety. A Regional coordination group conducted quality evaluation activities: monitoring the carrying out of the visits and promoting workshop among local groups about improvement actions. The team in charge of carrying out the visit is composed of representatives from the clinical risk management area, prevention and protection service and management. The team visits the healthcare facility and interviews the healthcare workers. It takes about one hour and all the healthcare workers are invited to participate. The information collected during the visit are included in a report, which is subsequently shared with the units. Proposals emerged from the visits may be used to inform improvement actions. After about six months, the team visits once again the facility to discuss on the improvement actions realized.
