Aim and the benefit of the Patient Safety Practice
 
The Pharmaceutical Department of the AUSL Bologna has started a project of clinical audit with the aim of optimizing the management of medication recognition, according to what defined in the Regional Recommendation n. 2/2011 and in the internal Procedure P094, to guarantee the safety in the prescription of medicines and the consequent appropriateness of care.
Description of the Patient Safety Practice
 
Clinical audit is a government tool that allows to appraise the adherence of clinical activity to the best available practices and it assures high standard of care. To this purpose, the Pharmaceutical Department ofn the AUSL Bologna has started a project of clinical audit. The multidisciplinary team that has been taking part in this project was composed by different professionals(pharmacists, doctors, nurses) belonging to: Pharmaceutical Department, Vascular Surgery department, Quality and Effectiveness department. The study population was made up by 250 patients hospitalized in the Vascular Surgery department with more than 24 hours of hospital stay. The strategy of data collection was retrospective and the period of observation was between September 2014 and January 2015.
Two reference criteria and their relative indicators have been identified: 1) presence of the recognition form in the medical record of the Vascular Surgery: 2) the medication recognition forms must be filled in and completed in every section. The standard related to indicator 1 has been reached in 100% of the cases, indeed in all the examined medical records the recognition form was present. As per indicator 2, 211 out of 250 forms have reached the standard criteria (80% of compiled fields).The use of the recognition form has allowed to identify areas of improvement that will be realized during the next phases of implementation and application of this form inside the AUSL of Bologna.
